Extended Care Facility

The Extended Care Facility at Boundary Community Hospital is a 20-bed licensed, skilled nursing home for long-term care with a 5 Star Rating and multiple Excellence in Caring Awards from the State of Idaho. The homelike environment of the facility and the family of caregivers who provide high-quality personalized care, are dedicated to the physical, mental, and emotional well-being of the residents.

The Extended Care Facility (ECF) is a residential setting that provides comprehensive medical and personal care services for individuals who require assistance with daily activities and have complex healthcare needs. The ECF is designed to cater to the needs of individuals who are unable to live independently due to chronic illnesses, disabilities, or the effects of aging in a safe, secure environment.

Services provided

  • Skilled nursing care includes registered and licensed nurses who manage medications, communication with physicians and coordination of specialized treatments and appointments.
  • Certified nursing assistants assist residents with their activities of daily living (ADLs) such as bathing, dressing, grooming, locomotion, toileting, and meals. Staff members are trained to handle various mobility needs and ensure the safety of residents.
  • Rehabilitation services that include physical therapy, occupational therapy, and speech therapy to help residents improve their functional abilities. These therapies aim to enhance mobility, strength, coordination, and communication skills.
  • Social and Recreational Activities staff encourage the residents to stay as active as possible through games, movies, walks, outings, jigsaw puzzles, arts and crafts, stretching exercises, and cultural events.
  • Registered dietitians assist the Nutritional Services manager with planning and oversight of resident meals to ensure they are well-balanced and meet the specific dietary needs and preferences of residents.
  • We offer specialized palliative and hospice services for residents requiring end-of-life care and support. These services focus on providing comfort, pain management, emotional support, and dignity during the final stages of life.
  • Local churches hold services every Sunday throughout the month per an alternating schedule. Several residents attend and enjoy the singing, the familiar hymns, and the short sermons.
  • ECF Care Coordinator/ designee assists residents and their families with discharge planning needs, emotional support resources available, community resources, and coordination of necessary appointments.

In 2018, Boundary Community Hospital’s ECF was recognized by the State of Idaho for providing excellence in resident care as a recipient of Gold L. Jean Schoonover Excellence in Caring Award. This was the eleventh Schoonover Award the facility received since 2004. This award is named in honor of L. Jean Schoonover, RN, who was the Chief of the Bureau of Facility Standards until her death in 1994. She is most remembered for her dedication and commitment to the assurance that the citizens of the state of Idaho residing in health care facilities received the highest quality of care possible.
