Patient-Centered Approach
Care Management at Boundary Community Hospital involves the patient, family, and caregivers, as well as our professional medical staff. This patient-centered approach is designed to assist in managing medical conditions, including chronic conditions and illnesses, more effectively, resulting in faster recovery and lower healthcare costs.
Assistance with Admission, Discharge Planning, and Referrals
The Care Management Department assists patients and their families with admission to the Extended Care Facility or the Swing Bed unit, discharge planning, and referrals to community and out-of-area resources.
Transition from In-Hospital Care to In-Home Care
Treatment does not end when you are discharged. The specialized Care Management team will help manage the transition from in-hospital care to in-home care. Focusing on what’s best for the patient, everyone from the Physician, Nurse, Rehabilitation Therapist, and Pharmacist collaborates to create a Discharge Care Plan. This plan evaluates the environment, resources, and support to ensure that each patient receives the best possible care.
Arranging Appointments and Functions
The department helps residents and patients arrange and attend appointments and functions outside Boundary Community Hospital and Extended Care Facility. The Care Manager functions as a patient advocate, Emergency Department grief counselor, and Acute Care case manager.
