Care ManagementCare Management at Boundary Community Hospital involves the patient, family and caregivers, as well as our professional medical staff. This patient-centered approach is designed to assist in managing medical conditions including chronic conditions and illnesses more effectively, resulting in faster recovery and lower healthcare costs.

The Care Management Department assists patients and their families with admission to the Extended Care Facility or the Swing Bed unit, discharge planning and referrals to community and out–of–area resources.

Treatment does not end when you are discharged. The specialized Care Management team will help manage the transition from in-hospital care to in-home care. Focusing on what’s best for the patient. Everyone from the Physician, the Nurse, the Rehabilitation Therapist, and the Pharmacist create a Discharge Care Plan that evaluates the environment, resources and support to ensure that each patient has the best possible care.

The department helps residents and patients arrange and attend appointments and functions outside Boundary Community Hospital and Extended Care Facility. The Care Manager functions as a patient advocate, Emergency Department grief counselor and Acute Care case manager.
