Department of Transportation (DOT) Physicals
Boundary Community Clinic is pleased to offer Department of Transportation (DOT) physical examinations for commercial drivers. Our certified medical examiners provide thorough evaluations in accordance with Federal Motor Carrier Safety Administration (FMCSA) guidelines to help ensure drivers meet the medical requirements necessary to safely operate commercial vehicles.
Whether you are obtaining a new Commercial Driver’s License (CDL), renewing your certification, or returning to work, our team is committed to making the process as convenient and efficient as possible. We understand the importance of keeping drivers healthy and on the road and strive to provide timely appointments and personalized care.
DOT physicals include a review of your medical history, vision and hearing screening, blood pressure assessment, and other evaluations required to determine medical fitness for duty. If additional information or follow-up is needed, our providers will work with you and your specialists to help you meet certification requirements whenever possible.

All DOT paperwork must be completed prior to arriving for the appointment. You are responsible for bringing your own forms to the visit (link below). If not completed, your appointment may be rescheduled. Required documents include

- Corrective lenses or hearing aids should be brought to the medical exam.
- Required documents as listed below for specific conditions.
The following medical conditions may require additional testing and/or follow-up prior to being issued a medical certificate. It is suggested that you see any specialists to clearly identify what is needed in letters regarding your medical condition/s. Required documentation includes:
- Diabetes:
- Hemoglobin A1C result (within past 3 months)
- If on insulin need 90 day glucose log
- Diabetic eye exam within the past year
- Treating PCP letter stating condition is stable and you are able to operate a commercial motor vehicle. This must be SPECIFCALLY stated in the letter.
- Heart Disease: (history or heart attack, stent placement, bypass, or other heart procedure:
- Treating cardiologist letter stating condition is stable and you are able to operate a commercial motor vehicle. This must be SPECIFICALLY stated in the letter.
- Annual cardiology examination with stress test results
- Pace Maker:
- Treating cardiologist exam and letter stating condition is stable and you are able to operate a commercial motor vehicle. This must be SPECIFICALLY stated in the letter.
- Congestive Heart Failure (CHF):
- Treating cardiologist exam and letter stating condition is stable and you are able to operate a commercial motor vehicle. This must be SPECIFICALLY stated in the letter.
- Evaluation must include copy of annual echocardiogram
- Stroke/TIA: If ever had a stroke or other condition that requires Coumadin or Warfarin
- PT/INR results (within past 3 months)
- Seizures: Please note that many seizure disorders are automatic disqualifiers for operating commercial motor vehicles.
- Letter from neurologist verifying you have been seizure free for 10 years and off seizure medication for 10 years and are able to operate a commercial motor vehicle. This must be SPECIFICALLY stated in the letter.
- Chronic Pain Syndrome:
- If you are prescribed any controlled medications such as narcotics, antidepressants, or sedatives, your provider must write a letter stating you have been informed not to take any narcotic or controlled substance within eight hours of driving.
- Sleep Apnea
- Letter from pulmonologist stating that you are compliant with treatment for you sleep apnea
- Copy of annual sleep study
- Insomnia
- If prescribed sleeping aids, your prescriber must write a letter stating you have been informed not to take this medication within eight hours of driving.
- Anxiety/Depression/Attention Deficit Disorder/Hyperactivity/Mental Health
- Any controlled substance prescription such as narcotics, antidepressants, or sedatives, your provider must write a letter stating you have been informed not to take any narcotic or controlled substance within eight hours of driving.
- Must also state condition is stable and you are able to operate a commercial motor vehicle.
The total fee for this service is $142.00 that must be paid PRIOR to the exam and is NOT billable to your insurance. Please call registration to make an appointment.
Some conditions may be discovered during the examination which may require further evaluation and testing at an additional expense. However you will only be charged the actual examination fee of $125 once. You will not be required to pay for an additional exam pending further workup if completed within 6 months of original exam. Any re-exam after 6 months will require a new DOT physical fee.
We do not provide drug screening with this exam as it is not a standard required by the Federal Motor Carriers Safety Administration but may be required by your company. If your company requires a drug test, it must be purchased separately from your DOT Physical exam cost.
