Welcome New Patients

Welcome to Boundary Community Clinics providing Primary Care to our Boundary County community.

As a new patient to the Clinic, a “New Patient Visit” will be scheduled. We ask all new patients to complete and return initial paperwork prior to the first visit.

  • Complete and return the forms to our offices at 6641 Kaniksu Street, Bonners Ferry 83805 or Fax (208) 267-3757.
  • Sign and return the Records Release

Ideally, in order to best prepare for your visit, we prefer that you return the completed forms to our office 24 hours prior to your appointment so that your chart can be created. Otherwise, please bring them with you to your first appointment.

Within the packet is the Records Release that you need to sign, so that we can request your medical records from your prior provider.  If we can get this form a few days early, it gives us the best chance to get your old records before the visit, to improve continuity of care.

On the Day of Your Appointment

Please arrive 15 minutes early and bring with you:

  • All of your prescriptions and supplements, in their bottles or packages with legible prescription labels
  • Your insurance card or billing information
  • Photo Identification

Payments:

Co-pays are due at the time of service. Co-pays or charges may be paid with cash, check, debit or credit card.

NOTE: The Clinic will request a list of medications, insurance/billing information and a photo ID at each visit.

Rescheduling an Appointment:

If you need to reschedule an appointment, please notify the Clinic at least 24 hours prior to the visit.
